Assisted Living

Assisted Living bridges the gap between independent living and nursing home care. Assisted Living typically offers assistance with the activities of daily living and ensures residents have access to staff 24 hours a day. The Assisted Living Waiver program is one of the long-term care options available statewide to Medicaid recipients and offers an alternative to nursing facility placement.

To be eligible for the Assisted Living Waiver program, you must be one of the following:

• Current nursing facility resident.
• Resident in an Ohio licensed residential care facility for at least six months.
• Consumer enrolled in PASSPORT/CHOICES or the Ohio Home Care Waiver.

Additionally, you must meet all eligibility requirements for enrollment in the waiver, including:

• You are age 21 or older.
• You are Medicaid eligible.
• You meet level of care at time of assessment 
• You have care needs that can be safely met in a residential care facility setting.
